Manifestations cliniques de la GEAE

classification établi par Klein et al. en 1970

• présentation clinique varie en fonction de la profondeur de l'infiltration dans la paroi digestive.
  • prédomine dans la muqueuse
    • signes cliniques
    • nausées
    • vomissements
    • douleurs abdominales
    • diarrhée avec malabsorption
    • parfois une entéropathie exsudative.
  • couche musculaire atteinte
    • syndromes obstructifs au premier plan.
  • atteinte séreuse prédominante
    • ascite riche en éosinophiles
    • occasionnellement à une effusion pleurale.

Actuellement : manifestations cliniques dépendent également de la distribution de l'infiltration à éosinophiles le long du tractus digestif.

• atteinte oesophagienne rarement symptomatique => dysphagie principal signe clinique.
• atteinte gastrique ( 40 à 80 %) antre => nausées, vomissements, douleurs abdominales et un amaigrissement.
• atteintes de l'intestin grêle plus fréquentes
  • associées à une autre localisation dans 75 % à 90 % des cas [14].
• atteintes duodénales => ulcérations ou sténose lorsque la musculeuse est atteinte.
  • tableau de diarrhée avec malabsorption et entéropathie exsudative.
• localisations coliques plus rares,
• ascite exsudative riche en polynucléaires éosinophiles [8, 25-26].
  • éosinophiles représentaient plus de 90 % des cellules péritonéales
  • (environ 400 à 140 000 éosinophiles/mm3).
  • à distinguer de l'hyperéosinophilie du liquide péritonéal survenant au cours
    • des dialyses chroniques [27],
    • des péritonites spontanées à Escherichia coli [28],
    • des lymphomes abdominaux [29],
    • de la périartérite noueuse
    • des cancers du pancréas [30].
• atteintes extra-intestinales
  • pancréatiques [32-36]
  • hépatiques [37-40]
  • vésicales [41]

Explorations biologiques

• hyperéosinophilie présente > 70 % des patients avec GEAE [6].
• dosages d'IgE totale élevés > 50 % des cas.

Explorations morphologiques

• explorations nécessaires
  • pour éliminer une atteinte extra-intestinale
  • affirmer le diagnostic de GEAE.
• diagnostic de GEAE essentiellement histologique => biopsies par voie endoscopique.
• aspect endoscopique => souvent normal.

Difficultés diagnostiques

• hyperéosinophilie intestinale,
• trois étiologies essentielles sont à éliminer :
  • les parasitoses,
  • les causes médicamenteuses
  • localisations intestinales d'un SHE [53].

• les parasitoses
  • éosinophilie modérée localisation ou cycle intestinal
    • taenia
    • trichocéphale
    • oxyure.
  • hyperéosinophilies importantes phase larvaire ou adulte dans d'autres tissus :
    • ascaridiose, toxocarose, trichinose, bilharziose, ankylostomose, anguillulose.
    • angiostrongylose intestinale (Angiostrongylus costaricensis)
    • Amérique Centrale, Amérique du Sud, Antilles.
    • => atteintes iléocaecales
  • ascite riche en éosinophiles => Toxocara canis [55].
  • colite granulomateuse => Strongyloides stercoralis [56, 57].
  • Ancylostoma caninum, parasite du chien =>grande majorité des GEAE en Australie [58],

• Tous les médicaments peuvent entraîner des manifestations allergiques avec hyperéosinophilie sanguine [59].
  • sels d'or [60],
  • hypoglycémiants oraux
  • psychotropes
  • antibiotiques [61]
  • anti-inflammatoires non stéroïdiens.
  • syndrome éosinophilie-myalgie tryptophane

• SHE se définit
  • présence d'une éosinophilie sanguine > 1 500/mm3
  • pendant plus de 6 mois
  • associée à des manifestations viscérales sans cause connue [64].
  • homme > femme.
  • atteinte du tube digestif (14 % des cas)
    • diarrhée avec malabsorption
    • hépatomégalie
    • splénomégalie [65].
  • distinction avec la GEAE :
    • (1) l'hyperéosinophilie sanguine constante et souvent majeure;
    • (2) l'atteinte multiviscérale qui peut être symptomatique ou silencieuse, dépistée par des examens complémentaires ;
    • (3) le dosage de l'IL-5 sérique [45].

Physiopathologie de la GEAE

Allergie ?

Traitements de la GEAE

mal définie en l'absence de protocole thérapeutique contrôlé et de la régression parfois spontanée de la maladie

• Dans les formes allergiques, => exclusion antigénique suffisante [88-90]
• épinéphrine pourrait être efficace [88].
• Corticothérapie => traitement d'attaque de 20-40 mg/j de prednisone pendant environ 15 jours, une diminution lente des doses peut-être obtenue.
• Si corticodépendance => 5-10 mg/j entretien
• corticoïdes d'action locale (budésonide) =>dose de 9 mg/j [94].
• corticorésistance => chromoglycate disodique [3, 6, 95] inconstants.
• anti-histaminiques (ketotifen ; 2-4 mg/j)
