Epidémiologie des hépatites chroniques B, C, D chez les patients VIH+

• Hépatites chroniques B
  • Quatre-vingts à 90 % des sujets infectés par le VIH ont des marqueurs sérologiques du VHB
  • 10 % sont porteurs chroniques de l'antigène HBs
  • Chez les toxicomanes intraveineux, il peut exister une co-infection avec le VHD.
• Hépatite chronique C
  • Plus fréquente des hépatites chroniques chez les malades infectés par le VIH.
  • Toxicomanes intraveineux et patients contaminés par des produits sanguins (transfusés, hémophiles)
  • Prévalence élevée de l'infection par le VHC
    • Facteurs de risque communs aux deux infections,
    • Taux élevé de passage à la chronicité de l'infection par le VHC dans ce contexte de déficit immunitaire
    • Test RIBA de 2e génération (31 %).
    • 82 % anticorps anti-VHC.
  • Chez les homosexuels non toxicomanes, la prévalence de l'infection par le VHC = 5 %
    • Faible transmission sexuelle de ce virus
  • Co-infection VIH-VHC susceptibles de transmettre le VHC par voie sexuelle.
  • La transmission materno-foetale du VHC paraît également favorisée par la coprésence du VIH
    • Evalué à 50 %
    • Cette transmission du VHC par des mères co-infectées par le VIH et le VHC est probablement en rapport avec un plus fort niveau de réplication du VHC, favorisé par la présence du VIH.
  • Groupe des toxicomanes intraveineux constitue le principal groupe à risque pour une, ou des co-infections par les virus des hépatites

Influence du VIH sur les infections chroniques B, B-D et C

• Virus B
  • Phase aiguë
  • Passage à la chronicité, significativement plus élevée que chez les sujets VIH-,
    • Patients homosexuels, prévalence du portage chronique de l'Ag HBs passe de 6 % à 20 %
    • Patients toxicomanes, prévalence du portage chronique de l'Ag HBs passe de 3 % à 89 %
  • Phase chronique
    • L'immunodépression, induite par le VIH, semble, en diminuant la réaction à médiation cellulaire dirigée contre les hépatocytes infectés, induire une tolérance immunitaire qui favoriserait la réplication du VHB.
      • Plus forte réplication, avec un taux moyen d'ADN du VHB sérique de 1 271 pg/ml contre 460 pg/ml dans le groupe VIH-
      • Etat d'immunodépression limite le conflit immunologique au niveau du foie, avec des lésions de nécrose hépatocytaire généralement modestes, et ce d'autant plus que l'évolution dans la maladie VIH est avancée.
      • Agressivité des lésions histologiques plus importante aux stades II et III du CDC, qu'au stade IV mais à confirmer.
      • Hépatites virales B cholestatiques et fibrosantes de mauvais pronostic ont été rapportées chez des patients infectés par le VIH
        • Comparables lors de la réinfection par le VHB après transplantation hépatique
        • Lésions pourraient être provoquées par une action cytopathogène directe du VHB
      • Réactivations rapportées, avec dans certains cas par une évolution fulminante avec cytolyse majeure chez malades profondément immunodéprimés, (CD4 < 50 éléments/mm3) au décours d'une corticothérapie ou d'une chimiothérapie
      • Disparition de l'Ag HBe moins souvent observée en cas de séropositivité VIH associée

• Virus D
  • Virémie D plus intense
  • Augmentation de la réplication virale B.
    • Chez les patients VIH-, le VHD exerce action inhibitrice sur la réplication du VHB, la présence du VIH lève cette inhibition
    • VHD cytopathogène se replique plus facilement à la faveur de l'immunosuppression induite par le VIH
      • Entraîne des lésions hépatiques plus sévères.
      • Evolutions fulminantes
        • Au cours de primo-infections,
        • Au cours d'infections chroniques

- Hépatite chronique C chez les malades infectés par le virus de l'immunodéficience humaine

Influence des virus hépatotropes sur l'infection VIH

• Effet Direct :
  • VHB pourrait accélérer la progression de la maladie VIH, mais données cliniques insuffisantes pour conforter cette hypothèse.
  • Coprésence du VHB et du VIH dans les cellules mononucléées du sang périphérique de malade
    • Chez les homosexuels, la prévalence de l'ADN du VHB dans les cellules mononucléées périphériques
      • Quasi constante
      • Indépendante du niveau de réplication virale ou du stade de la maladie VIH
    • Chez les toxicomanes, la prévalence de la présence d'ADN du VHB dans ces cellules apparaît beaucoup plus faible,
      • N'excédant pas 18 %
      • Corrélée au niveau de réplication virale.
  • Différence majeure entre homosexuels et toxicomanes, également observée chez les sujets VIH- pourrait être expliquée par la coprésence, chez les toxicomanes, du VHC. Le VHC pourrait intervenir en empêchant l'infection des cellules mononucléées par le VHB, ou en réprimant la réplication du VHB dans ces cellules
• Effet indirect :
  • Infection chronique par le VHB entretient la production de cytokines, qui pourraient à leur tour stimuler la réplication du VIH.

Diagnostic

Clinique:

• Asymptomatiques (dosage de transaminases, sérologie d'hépatite).
• Asthénie d'intensité variable.
• Symptomatologie plus bruyante (ictère).
• Stade de cirrhose par des signes d'insuffisance hépato-cellulaire ou d'hypertension portale

Biologiquement

• Elévation des transaminases plus modérée que chez les sujets VIH-
• Au cours de l'hépatite C fluctuations des transaminases avec des périodes de normalisation plus ou moins prolongées
• Forme cholestatique essentiellement marquée par une élévation de la gamma GT
  • Pour certains auteurs = patients à un stade avancé de la maladie VIH (taux de CD4 < 150 ou SIDA)
  • Indispensable d'éliminer d'autres pathologies
    • Infection à mycobactérie
    • Infection à cytomégalovirus
    • Cholangite.
  • Chez les patients ayant une co-infection VIH-VHB
    • Tolérance clinique, biologique et histologique est souvent observée, en dépit d'une forte réplication virale
    • Pas corrélée au taux de lymphocytes CD4
    • Ponction biopsie hépatique est nécessaire pour confirmer le diagnostic d'hépatite chronique et apprécier l'importance des lésions.
    • D'autres anomalies biologiques
      • hypoalbuminémie
      • hypergammaglobulinémie
      • thrombopénie
    • Concernant les tests sérologiques spécifiques, des problèmes d'interprétation peuvent se poser pour l'hépatite C.
      • Les tests de première génération = résultats faussement positifs en raison de l'hypergammaglobulinémie
      • Intérêt de la détection d'une virémie C par PCR

Aspects thérapeutiques

Prévention et traitement des hépatites B

• Vaccination
  • Réponse significativement moins bonne que chez les sujets immunocompétents
  • Amélioration significative du taux de séroconversion à l'aide d'un protocole comportant 4 injections (40 µg/dose) de vaccin recombinant (à 0, 1, 2 et 6 mois)
• Traitements
  • Antirétroviraux
    • Marcellin et al. [52] : VIH+ traités par AZT (600 à 1 200 mg/j) : pas eu de réduction significative des taux d'ADN (mais stade de SIDA)
    • DDI : pas d'effet significatif [53, 54].
    • Lamivudine semble par contre plus intéressante [55].
  • Vidarabine
    • Absence de réponse à la vidarabine chez patients homosexuels et VIH +
  • Interféron alpha
    • Pas de réponse si utilisé seul [55].
    • IFN alpha en association avec la zidovudine : Annulation de l'ADN du VHB dans 1/3 des cas, pour les doses les plus fortes d'IFN alpha (6 MU) [57].
    • Pas de corrélation significative entre réponse et stade de la maladie VIH (nombre de lymphocytes CD4).
    • Chute des lymphocytes CD4 possible avec infections opportunistes [58]

Traitement des hépatites B-D

• IFN alpha recombinant : efficacité avec des doses de 9 millions 3 fois/semaine pendant 6 semaines (malade VIH peu avancée [59].
