• Episodes de reflux
• Non II à un tonus de base du sphincter inférieur de l'oesophage (SIO) trop faible,
• mais II relaxations du SIO pas provoquées par des déglutitions [1].
  • Relaxations transitoires du SIO (RTSIO)
    • Existe chez les sujets sains
    • expression manométrique de l'éructation [2].

Diagnostic des RTSIO

relaxations du SIO qui ne sont pas provoquées par une déglutition.

Enregistrement du tonus du SIO

• utilisation de sondes équipées d'un manchon perfusé (sleeve)
• pas perturbé par les variations de la position du SIO pendant l'examen [4].
• autre modèle de sonde (reverse perfused sleeve) permet améliorerpropriétés,
• intégrées des électrodes enregistrement de l'activité électromyographique du diaphragme [5].

Détection des déglutitions

• Deux techniques peuvent être utilisées :
  • la manométrie
    • capteur de la sonde de manométrie
    • telle sorte que les ondes pharyngées soient enregistrées
  • EMG mylohyoïde
    • électrodes de surface placées sous le menton.
• relaxation du SIO
  • entre 2 s avant et 4 s
  • après le début de la contraction pharyngée,
  • 3 s après le début du complexe EMG mylohyoïde [3, 6].

Durée de la relaxation

• relaxations induites par les déglutitions
  • durent en moyenne 6 s à 8 s
• RTSIO
  • durent presque toujours plus de 10 s
  • parfois jusqu'à 30 s à 45 s [3, 6, 7].

Autres critères

• vitesse relaxation supérieure ou égale à 1 mmHg/s
• relaxation complète obtenue en moins de 10 s [3].
• pression résiduelle observée au cours de RTSIO doit être < 2 mmHg.

RTSIO et reflux gastro-oesophagien

Reflux physiologique

• RTSIO à l'origine de 80 % à 100 % des épisodes de RGO physiologique [1, 8, 9].
• Autres circonstances de survenue des reflux
  • Augmentation de la pression abdominale
  • Reflux dits spontanés ( SIO très hypotonique)

Reflux pathologique

• RTSIO représentent
• 65 % et 82 % des mécanismes de reflux [1, 9, 11].
• Mais
  • plus le grade de l'oesophagite est élevé
  • plus la proportion de reflux spontanés [11].
• Eructations associées
  • 67 % des reflux en cas d'oesophagites de grades 0-1,
  • 35 % pour les grades 2-3
  • 13 % pour les grades 4 [12]
• Reflux provoqués par une hyperpression abdominale observés chez 2 % des témoins,
  • 15 % des patients avec oesophagite de grades 0-1
  • 40 % pour les grades 2-3
  • 22 % pour le grade 4
• RTSIO mécanisme essentiel de reflux chez l'homme.
  • 35 % à 50 % des RTSIO sont accompagnées d'un reflux chez les sujets sains
  • contre 60 % à 70 % chez les patients [1, 9, 13].
