complications de la lithiase vésiculaire

• cholécystite aiguë,
• cholécystite chronique,
• lithiase du cholédoque
• pancréatite aiguë.
• symptômes à tort, attribués à la lithiase :
  • dyspepsie,
  • colopathie fonctionnelle,
  • migraines,
  • céphalées diverses.
  • fréquence identique en présence ou en l'absence de lithiase [2-4].
• cancer vésiculaire : 0,01 % à 0,02 % par an [6].

Faut-il proposer une cholécystectomie ?

• Balance,
• morbidité mortalité lithiase vésiculaire asymptomatique,
• morbidité et la mortalité cholécystectomie traditionnelle,
  • cholécystectomie préventive systématique - aucun bénéfice
• Pas de bénéfice
  • diabète,
  • une insuffisance coronarienne,
  • insuffisance rénale.
• Exception: vésicule porcelaine (ou calcifiée) =>risque élevé de cancer vésiculaire.
• justifie une cholécystectomie systématique, sauf personne très âgée.

Depuis coelioscopie

=>augmentation 30 % du nombre global de cholécystectomies

=> élargissement des indications
