Foie et VIH

• VIH pas un virus hépatotrope mais constatations cliniques et biologiques suggèrent une atteinte hépatique directe par le virus.
• Anomalies histologiques hépatiques « inhabituelles » fréquemment constatées
  • hyperplasie kupfferienne
  • péliose [1].
• Anomalies des tests hépatiques au cours des primo-infections à VIH [2] est également en faveur d'une telle hypothèse.
• Cellules de Kupffer et cellules endothéliales exprimant le marqueur CD4 peuvent héberger le VIH.
• Modèles d'étude in vitro ont montré l'influence du VIH sur la sécrétion des cytokines par les cellules de Kupffer ainsi que sur la production des facteurs de coagulation par les cellules endothéliales [3].

Co-infections par le VIH et hépatites virales

• 1/3 des malades : au moins 1 marqueur sérologique d'hépatite virale.
• 1/6 : a hépatite chronique virale B et/ou C.

Co-infections par le VIH et hépatites virales B

• Forte séroprévalence avec HVB
  • VHB même modes de transmission (parentérale, sexuelle).
  • AgHBs ou Ac anti-HBs et/ou anti-HBc présents
    • > 80 % des toxicomanes
    • > 75 % des homosexuels infectés par le VIH [4, 5]
  • Risque accru d'infection par le VIH (OR de 1,3 à 2) chez les malades infectés par le VHB [7].
• Réponse immunitaire moins bonne -> taux faibles ou nuls d'anticorps anti-HBs et moindre fréquence de la séroconversion HBe [10].
• Portage chronique de AgHBs et risque de développement d'une hépatite chronique plus élevé (10 %) [11]
• Réplication virale B (ADN du VHB) en moyenne plus élevée
• Evolution vers la cirrhose plus rapide
• Mais cirrhoses virales B rares
• L'allongement de l'espérance de vie modifieront probablement ces données.
• Fréquence des profils sérologiques atypiques
  • Disparition Ac anti-HBs et/ou Ac anti-HBc
  • Réactivation virale B spontanée , avec réapparition de Ag HBe et parfois réapparition de Ag HBs [15, 16].
  • Réactivations -> hépatites graves (cholestasiante et fibrosante)

Co-infections par le VIH et hépatites virales D

• 50 % des toxicomanes infectés par le VIH et le VHB [14].
• Co-infection VHD et du VHB -> hépatite fulminante.
• Surinfection favorise-> hépatite chronique B + D [19].
• Chez co-infectés B, C et D -> gravité
  • VHD cytopathogène
  • Immunodépression induite par le VIH augmente sévérité de l'atteinte hépatique en favorisant la réplication du VHD

Co-infections par le VIH et hépatites virales C

• Prévalence des anticorps anti-VHC chez toxicomanes : 75 % [20].
• Prévalence des anticorps anti-VHC chez homosexuels > population générale (4 % à 8 %) [21].
• Transmission materno-foetale du VHC augmentée d'un facteur 2 chez les malades co-infectés par le VIH [22].
• Formes graves cholestasiantes et fibrosantes observées,
• Evolution vers la cirrhose plus rapide (moyenne de 8 ans [25], contre 15 ans en l'absence de co-infection par le VIH).
  • Licite de réaliser un contrôle histologique tous les 3 ans.
  • La co-infection par le VHB et consommation d'alcool associées à une progression plus rapide vers la cirrhose [26].
• Diagnostic d'hépatite chronique C
  • Recherche des anticorps par un test Elisa 3.
  • Tests faussement négatifs +++
  • Détection du génome du VHC (ARN viral)
  • Charge virale du VHC plus élevée - facteur prédictif de mauvaise réponse au traitement par interféron [27].
  • Génotype 3a prépondérant.

Co-infections par le VIH et hépatites virales G

• Prévalence du VHG > 30 % [29].

Infections opportunistes hépatiques

• L'atteinte histologique la plus fréquente ->
  • Lésions granulomateuses focalisées
  • Atteinte diffuse parenchymateuse.
• Biochimique ->
  • Cholestase variable
  • Hypertransaminasémiemodérée.

• Infections à mycobactéries
  • Mycobacterium avium complex ou MAC
  • Mycobacterium tuberculosis
    • Principale cause
    • Diagnostic : prélèvements pulmonaires ou ganglionnaires
    • Hémocultures sur Isolator® ont une sensibilité supérieure à 80 % [37].
• Hépatite à cytomégalovirus (CMV)
  • Rarement symptomatique.
  • Cholestase fébrile avec hypertransaminasémie modérée.
  • Histologique du foie : granulome -> présence d'inclusions nucléaires au niveau des cellules de Kupffer, des cellules endothéliales et des hépatocytes [41].
  • L'hybridation in situ peut être d'une aide précieuse.
  • Le traitement par ganciclovir ou foscarnet permet une régression rapide des anomalies hépatiques,
• L'hépatite herpétique,
  • Tableau d'hépatite fébrile avec hypertransaminasémie très importante (50 à 150 fois la normale).
  • Histologie réalise des foyers de nécrose hémorragique [42].
  • Traitement par l'aciclovir efficace mais le pronostic sévère.
• Infections disséminées mycotiques
  • cryptocoque
  • histoplasme
  • Candida
    • Asymptomatiques.
    • Histologie est celui de granulomes pouvant évoquer une lésion de tuberculose.
• pneumocystose et la toxoplasmose
  • Exceptionnelle
  • Pneumocystoses hépatiques responsables lésions des sinusoïdes hépatiques contenant des amas de kystes (visualisés par la coloration argentique).
  • Traitement : cotrimoxazole
• L'incidence des atteintes viscérales à Leishmania infantum est croissante en zone d'endémie.
• L'agent de l'angiomatose bacillaire (Bartonella henselae) peut être responsable de lésions hépatiques à type de péliose [45] posant alors un problème diagnostique avec le sarcome de Kaposi [46]. Après identification de la bactérie causale par coloration de Warthin-Starry, un traitement par macrolides ou tétracyclines paraît efficace. Les hépatites bactériennes (listériose) ne semblent pas plus fréquentes que chez les malades non immunodéprimés.

Lymphomes et sarcome de Kaposi

• Sarcome de Kaposi
  • Fréquents (15 % dans une série autopsique) [42].
  • Asymptomatiques
  • Découvertes sur des examens morphologiques (échographie, scanner)
    • Tumeur volumineuse parenchymateuse
    • Tumeur hilaire à extension vasculaire.
• Lymphomes
• Les localisations hépatiques des lymphomes plus fréquentes chez les patients atteints de sida [42].
• Lymphomes B de haute malignité (tardif)
• Lymphome de Burkitt (très précoce).

Autres atteintes hépatiques

• Atteintes médicamenteuses
  • Incidence croissante avec l'évolution de la maladie.
  • Polymédication des malades
  • Grande fréquence des complications allergiques (cotrimoxazole [48]).
• Stéatose macrovacuolaire
  • Très fréquente dans l'infection par le VIH [49],
  • Intoxication alcoolique,
  • Infection par le VHC
  • Dénutrition
  • Anomalies du métabolisme lipidique [50]
• Syndrome d'hémophagocytose
  • Tableau fébrile associant hépato- et splénomégalie.
  • Bi-, voire une pan-cytopénie.
  • Augmentation de l'activité sérique de la gamma-GT et des transaminases
  • Hypertriglycéridémie
  • Hyperferritinémie

Atteintes biliaires au cours de l'infection par le VIH

• Formes symptomatiques(0,2 % à 2 % ).
• Cause infectieuse 2/3
  • cryptosporidies
  • microsporidies,
  • Cyclospora sp
  • Isospora belli
  • cytomégalovirus
• Cholangites
  • Clinique
    • Douleurs
    • Asymptomatiques (10 % à 30 % ).
    • Ictère très rare.
    • TAngiocholite
  • Biologiquement,
    • Cholestase d'aggravation progressive
    • Hypertransaminasémie modérée
    • Parfois augmentation enzymes pancréatiques.
  • Echographie
    • Dilatations des voies biliaires intra-hépatiques ou de la voie biliaire principale
    • 25 % épaississement paroi voies biliaires.
    • Aspect proche cholangite sclérosante primitive avec des images de sténose localisée associée à des dilatations segmentaires.
  • Echo-endoscopie plus sensible
  • Opacification rétrograde des voies biliaires ssi sténose du bas cholédoque,
  • Seule une restauration prolongée de l'immunité semble limiter les récidives de ces affections chroniques.
• En pratique, devant la découverte d'une cholangite asymptomatique ou pauci-symptomatique
  • Simple surveillance biologique et échographique
  • Si douleurs importantes et/ou Complications infectieuses-> CPRE
  • Sténose longue du bas cholédoque -> pose d'une endoprothèse ou chirurgie
• Tableau de cholécystite alithiasique
  • Cholécystectomie -> régression des douleurs mais récidive dans le cadre d'une cholangite diffuse associée.
  • Indication opératoire ssi
    • Tableau clinique aigu avec défense pariétale
    • Refroidissement par antibiotiques inefficace.
