La rupture de varices sophagiennes (VO) : 70 % des causes des hémorragies digestives au cours des cirrhoses.

La ligature de VO permet

• Amélioration de la survie
• Nombre moindre de complications après ligature qu'après sclérose
• Nombre moindre de récidive hémorragique
• Mais bênéfice de la ligature sur la survie des malades n'a jamais été clairement établi

Ligature de VO essentielle dans la prévention secondaire de la rupture de VO associée à un traitement médical par bêtabloquants

Physiopathologie

La ligature agit par strangulation de la varice, induisant une obstruction veineuse et une stase qui provoque la thrombose puis la fibrose du vaisseau.

Incidence des complications

Incidence des complications après ligature de VO varie de 2 % à 44,7 % et reste significativement moins importante qu'après sclérose de VO

Complications liées à la technique

Il y a quelques années, ces complications étaient liées principalement au « sur-tube » utilisé pour permettre le passage répété de l'endoscope et actuellement abandonné depuis l'avènement des kits multi-élastiques.

Risque du « sur-tube »

• pincement sans gravité
• ulcération de la muqueuse
• perforation eosophagienne (incidence 0 % à 16,4 %)

Actuellement :

• Incidence des pneumopathies après ligature de VO (0 % à 5 % )
• Désaturation en oxygène fréquente (23 % saturation en oxygène <90 % )
  • Il faut prescrire à chaque malade une oxygénothérapie de 2 l/min.

Complications locales

• Ulcères post-ligature de VO (2 % à 100 %)
  • Superficiels (environ 0,6 mm de profondeur)
  • Limites nettes, larges (85 mm2)
  • Jaunâtres
  • Cicatrisent en moyenne en 14 jours
  • asymptomatiques
  • Découverts lors de l'endoscopie suivante
  • Aucune prise en charge particulière.
  • Diminution de l'incidence des ulcères après ligature de VO possible chez les malades recevant de l'oméprazole
• Ulcères eosophagiens
  • Parfois hémorragiques
  • Fréquence (4 % à 9 %)
  • Plus profonds
  • Gravité de l'hémorragie modérée
  • Parfois transfusions ou tamponnement (sonde de Blakemore)
  • Diagnostic : fibroscopie élimine une récidive de la rupture de VO.
• Dysphagie post-ligature
  • durée de 24 à 48 heures
  • Incidence de 0 % à 24,3 %
    • Obstruction eosophagienne par protusion des varices liées déclenchée lors de la reprise alimentaire
      • douleur thoracique
      • hypersialorrhée
    • Spasme eosophagien
      • dysphagie transitoire
    • Sténose eosophagienne.
  • Prise en charge est symptomatique
    • régime semi-liquide
    • antalgiques
    • sucralfate
    • inhibiteurs de la pompe à protons
    • reprendre l'alimentation orale par un repas semi-liquide le soir de l'examen.
• Anomalies de la motricité eosophagienne
  • pas de modification de la relaxation du sphincter inférieur de l'eosophage
    • de l'amplitude
    • de la durée
    • de la vitesse de propagation des ondes de contraction eosophagienne
• Sténoses eosophagiennes après ligature de VO
  • (0 % à 4 % )
• Hématome sous-muqueux est une complication exceptionnelle de la ligature de VO
• Perforation eosophagienne exceptionnelle par nécrose de la muqueuse décrits chez des malades traités par immunosuppresseurs

• Douleurs rétrosternales (1,5 % à 24,3 % )
  • Douleurs modérées,
  • Régressent en 24 à 48 heures
  • Pas de prise en charge particulière.
• Augmenterait le risque d'apparition à moyen terme d'une gastropathie d'hypertension portale ou de varices cardio-tubérositaires
  • Modification du flux sanguin gastrique par blocage du flux gastrique sortant

• Hyperthermies en rapport avec la ligature de VO en prévention de la récidive hémorragique ( 2,2 % à 24,3 % )
• Complications infectieuses décrites après ligature de VO pour prévention de la récidive hémorragique
  • Pneumopathies (le plus souvent d'inhalation)
  • Infections du liquide d'ascite (0 % à 16 %)
    • incidence plus élevée qu'au cours des scléroses de VO
    • proportionnelle à la gravité de l'hépatopathie
• Hémorragie d'origine rectale
  • Réduction du shunt coronaro-azygos et développement d'autres voies de dérivation porto-cave, en particulier une connexion entre la veine mésentérique inférieure et la veine iliaque gauche.
• Thrombose de la veine mésentérique supérieure
• Modification du flux portal entraînant la thrombose des veines de la sous-muqueuse induisant alors une stase, puis une thrombose du réseau veineux mésentérique
