Source d'infection et facteurs de risque

B. henselae peut être à l'origine d'une lymphadénite (classique maladie des griffes du chat), d'une angiomatose bacillaire ou d'une péliose du foie et de la rate chez les malades immunodéprimés, de bactériémie chronique et d'endocardite chez les personnes âgées

Cette bactérie est transmise à l'homme par griffure ou morsure d'un jeune chat bactériémique mais asymptomatique.

Atteinte hépatique au cours des infections par B. henselae

• Infection par B. henselae,
• Deux principaux types de lésions hépatiques peuvent être observés :
  • Granulomatose nécrosante malades immunocompétents et chez les immunodéprimés,
  • Lésions de péliose immunodéprimés, essentiellement au cours du sida [13].

* Chez le malade immunocompétent

• Lésion cutanée au point d'inoculation + adénopathie locale.
• 1 à 2 % se compliquent d'une atteinte systémique [17].
• Localisation hépatique
  • Multiples
  • Aaspect de nodules bien circonscrits,
  • 4 à 10 mm,
  • Hypoéchogènes à l'échographie et
  • Hypodenses au scanner.
  • Exceptionnellement se calcifier.
• Adénopathies multiples, superficielles et profondes
• Histologiquement,
  • granulomes, nécrosés, infiltrat leucocytaire,
  • histiocytes en palissade
  • cellules géantes et parfois des abcès [18].
  • granulomes peuvent être caséeux.
• diagnostic facile si: -> griffure de chat, adénopathie,
• difficile -> fièvre associée masses intra-hépatiques et spléniques
  • interprétées métastases ou des localisations de lymphome.
  • diagnostic qu'après laparotomie [19].

* Chez le malade immunodéprimé

• Péliose
• Hépatalgies,
• Hépatomégalie + splénomégalie parfois volumineuses.
• pas d'ictère
• Altération profonde de l'état général
• Fièvre, frissons, sueurs, peut être au premier plan,
• 40 % lésions d'angiomatose bacillaire peuvent, être confondues avec un sarcome de Kaposi,
• Risque hémorragique dilatations péliotiques -> biopsie doit être dirigée par échographie ou faite par voie transveineuse.

Les éléments diagnostiques

• Taux élevé d'anti-IgG spécifiques
• Taux diminuant dans l'année qui suit le diagnostic.
• Immunofluorescence
  • sensibilité de 98 %
  • spécificité de 95 % [11].
• Anatomopathologique
  • coloration standard (hématoxyline-éosine)
    • visualiser les granulomes
    • lésions de péliose
    • ne montre pas directement le germe.
  • Dépôts granuleux amorphes à des amas de bactéries, de polynucléaires et de macrophages.
  • La coloration argentique de Warthin-Starry visualise les bacilles
  • peut être négative en cas de prélèvement tardif
  • coloration n'est pas spécifique
    • Nocardia,
    • Helicobacter
    • Legionella
    • Whipple
    • spirochètes.
• L'amplification génique (PCR)

Le traitement

• Malade immunocompétent
  • Traitement pas nécessaire dans les formes classiques, locales
  • Dans les formes diffuses avec localisations viscérales,
    • érythromycine ou doxycycline
    • au moins un mois
• Malade immunodéprimé,
• érythromycine (2 à 4 g par jour) par voie orale ou intraveineuse et la doxycycline (100 mg deux fois par jour) par voie orale sont efficaces
• ltétracycline et minocycline.
